In providing these Services, we act as a “business associate” (as defined under the Health Insurance Portability and Accountability Act (“HIPAA”)) when we collect and process Personal Information that constitutes “protected health information” (as defined by HIPAA) (“PHI”) on behalf of our “covered entity” (as defined by HIPAA) clients. This Privacy Policy also explains your choices for managing your information preferences, including opting out of certain uses of your Personal Information (defined below). This Privacy Policy applies to all users of the Site and Services. The Site and Services are provided by RNK Health LLC (“Company,” “we,” “our,” “us”).

Personal Information We Collect
We collect the following categories and types of “Personal Information”:
- Contact Information: your first and last name, mailing address, email address, and phone number;
- Other identifying information: IP address, advertising ID, device ID, social media user names, passwords, and other security information for authentication and access;
- Internet or other electronic activity: your browsing and click history, including information about how you navigate within our Site and Services and which elements of our Site and Services you use the most;
- Commercial information: products or services purchased or viewed on our Site; and
- Inferences drawn from the categories described above to create a profile about you to reflect your preferences, characteristics, behavior, and attitude.
Our Company takes the privacy and security of Protected Health Information (PHI) seriously and complies with all HIPAA requirements. PHI is protected through encryption, secure storage, and access controls, ensuring that only authorized personnel can access it. PHI is shared only with the parties necessary to provide your care and fulfill your order, including our medical partners and the professional entities that employ or contract the licensed providers who treat you — such as OpenLoop Health, Inc. and M & D Integrations, Inc. (DBA MD Integrations) and their affiliated professional entities — as well as the licensed third-party pharmacies that prepare and ship any prescribed medication. Which partners receive your PHI depends on the specific product and the state in which you are treated. Each such partner is bound by HIPAA and/or contractual obligations to safeguard your PHI.

How We Use Personal Information
We use your Personal Information for the following categories of use:
Transactional Purposes
We use your contact information, other identifying information, and commercial information to receive, confirm, and process your order or account registration; to provide the Services; and to communicate with you about your account.
Your access fees provide immediate access to our clinical team and are billed based on the selected term (e.g., monthly, quarterly, or annually). To receive your next medication shipment, you may be required to complete a periodic refill or follow-up form. Your access remains active, and access fees will continue to be billed while you complete this step.
Analytical Purposes
We use your other identifying information, internet activity and browsing history, and commercial information to analyze preferences, trends, and statistics.
Marketing and Promotional Purposes
We use your contact information, commercial information, internet or other electronic activity, and inferences to inform you of our new products, services, and offers; provide you with targeted advertising; and provide you with other information from and about our Company, including personalized marketing communications.
Maintenance and Improvement of the Site and Services
We use your contact information, commercial information, and internet activity and browsing history to provide you with the Site and Services, including sending you alerts about your account; handle your customer service requests; and help us diagnose technical and service problems and administer the Site and the Services.
Security and Fraud Prevention
We use your contact information, other identifying information, commercial information, internet activity and browsing history, and inferences to protect the Site and Services, our Company, and others, and to prevent fraud, theft, and misconduct.

Your Rights Regarding Health Information
Get a Copy of Your Medical Record
You can request an electronic or paper copy of your medical record and other health information we have about you. We will provide a copy or summary within 30 days of your request, with a reasonable, cost-based fee.
Request to Correct Your Medical Record
You can ask us to correct health information that is incorrect or incomplete. We may deny your request, but we will explain why in writing within 60 days.
Request Confidential Communications
You can ask us to contact you in a specific way (e.g., home or office phone) or to send mail to a different address. We will accommodate all reasonable requests.
Ask Us to Limit What We Use or Share
You can ask us not to use or share certain health information for treatment, payment, or operations. We are not required to agree, but we will consider your request. If you pay for a service in full, you can ask us not to share that information with your health insurer, and we will comply unless a law requires otherwise.
Get a List of Those with Whom We’ve Shared Information
You can ask for a list of times we’ve shared your health information over the past six years, including who we shared it with and why. We provide one free accounting per year; a reasonable, cost-based fee will apply for additional requests within 12 months.
Get a Copy of This Privacy Notice
You can request a paper copy of this privacy notice at any time, even if you have agreed to receive it electronically. We will provide it promptly.
Choose Someone to Act for You
If you have given someone medical power of attorney or have a legal guardian, that person can exercise your rights regarding your health information. We will confirm their authority before taking action.
File a Complaint if You Feel Your Rights Are Violated
You can file a complaint if you believe we have violated your rights by contacting us directly. You may also file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights. We will not retaliate against you for filing a complaint.
Categories of Third Parties We Share Personal Information With
Service Providers
From time to time, we may establish a business relationship with other businesses whom we believe trustworthy and who have confirmed that their privacy practices are consistent with ours (“Service Providers”). For example, we may contract with Service Providers to provide certain services, such as hosting and maintenance, data storage and management, and marketing and promotions. We only provide our Service Providers with the information necessary for them to perform these services on our behalf. Each Service Provider must agree to use reasonable security procedures and practices, appropriate to the nature of the information involved, to protect your Personal Information from unauthorized access, use, or disclosure. Service Providers are prohibited from using Personal Information other than as specified by us.
Medical Partners and Pharmacies
To provide your care, we share PHI with the professional entities that employ or contract your treating providers (including OpenLoop Health, Inc. and M & D Integrations, Inc. and their affiliated professional entities) and with the licensed third-party pharmacies that prepare and ship your medication. These partners use your information solely to provide clinical care, dispense and deliver medication, and meet their legal and regulatory obligations.
Third Parties
Mobile information will not be shared or sold with third parties for marketing purposes. We may share Personal Information with a third party if you specifically authorize it, or if it is required in order to complete a transaction, service, or activity that you initiated or requested. In such a case, the collection and processing of your Personal Information may be subject to the privacy policy of such third party.
Our Affiliates
Mobile information will not be shared or sold with affiliates for marketing purposes. We may share Personal Information with businesses controlling, controlled by, or under common control with our Company.
Corporate Transactions
If our Company is merged, acquired, or sold, or in the event of a transfer of some or all of our assets, we may disclose or transfer Personal Information in connection with such transaction. You will have the opportunity to opt out of any such transfer if, in our discretion, it will result in the handling of your Personal Information in a way that differs materially from this Privacy Policy.
Compliance with Laws and Law Enforcement
We cooperate with government and law enforcement officials and private parties to enforce and comply with the law. You can also opt out of receiving marketing e-mails from us by following the link provided at the bottom of all marketing e-mails you receive from us. You are able to opt out of receiving marketing e-mails from us; however, you cannot opt out of receiving all e-mails from us, such as e-mails about the status of your transactions or account. If you have questions or concerns regarding this Privacy Policy, please e-mail us at that same address.

Please be aware that whenever you share information on any public area of the Site or Services, that information may be accessed by others. Children’s Privacy
The Site and Services are intended for users who are 18 years old or older. We do not knowingly collect Personal Information from children under the age of 13. If we become aware that we have inadvertently received Personal Information from a child under the age of 13, we will delete such information from our records. California users under the age of 18 may request the removal of their content or information publicly posted on the Site or Services by e-mailing us at support@myrnk.com.
Processing in the United States
Please be aware that your Personal Information and communications may be transferred to and maintained on servers or databases located outside your state, province, or country. If you are located outside of the United States, please be advised that we process and store all information in the United States. The laws in the United States may not be as protective of your privacy as those in your location. By using the Site or Services, you are agreeing that the collection, use, transfer, and disclosure of your Personal Information and communications will be governed by the applicable laws in the United States.

We do not share your Personal Information with third parties for their direct marketing purposes.
